We have a Congregate Living Health Facility CLHF for sale in the Torrance, South Bay, LA County, CA that is turn key ready, meaning that the licensing and the facility are brand new and ready to be operated. This facility took three years from its inception to where it is now with its 'A' and its 'B' license in place. The facility has an active Medi-Cal provider number with the facility actively able to work with Medi-Cal insured patients.

The current property consists of a lot that is 5,800 square feet and a home that is 1500 square feet; completely renovated and brought up to compliance for a facility. Congregate Living Health Facilities (CLHFs) are defined in H&S Code, Section 1250(i) (1), as a residential home with a capacity of no more than six beds, which provides inpatient care, including the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, social recreational, and at least one type of service specified in paragraph (2). The primary need of CLHF residents shall be for availability of skilled nursing care on a recurring, intermittent, extended, or continuous basis. The care is generally less intense than that provided in general acute care hospitals but more intense than that provided in skilled nursing facilities.

This facility currently has a CLHF (A) license which means it provides services to persons who are mentally alert, physically disabled, who may be ventilator dependent.

This facility currently has a CLHF (B) license. Services for persons who have a diagnosis of a terminal illness, a diagnosis of a life-threatening illness, or both. Terminal illness means the individual has a life expectancy of six months or less as stated in writing by his/her attending physician and surgeon. A 'life-threatening illness' means the individual has an illness that can lead to a possibility of a termination of life within five years or less as stated in writing by his or her attending physician and surgeon.

The ideal buyer would know how to operate the business portion of the CLHF to include understanding the compliance issues, the day to day operational issues to include marketing, staffing, patient related logistics.

The seller can provide the buyer with a list of qualified Medicare consultants that can help the new owner with the Change of Ownership (CHOW) paperwork.
